MONOFIX-VF (Clotting Factor IX Concentrate)
Indications for Use
MonoFIX-VF is indicated for the treatment of haemorrhages, for use in surgery, and as prophylaxis in patients with haemophilia B. MonoFIX-VF is not indicated for the treatment of factor II, VII or X deficiencies because it does not contain therapeutic levels of these coagulation factors. MonoFIX-VF is not indicated for the treatment of haemophilia A patients with factor VIII inhibitors. Haemophilia should be managed in consultation with a specialist Haemophilia Treatment Centre.
Availability
Usually issued on an individual patient basis following approval by an ARCBS Haematologist/ Medical Officer. Hospitals may request supplies for stock from ARCBS. The Haemophilia centres and ARCBS work closely together to optimise patient care.
MonoFIX-VF is currently available in 500 IU and 1000 IU vial sizes.
Precautions
- Although MonoFIX-VF contains purified factor IX, the potential risk of thrombosis and DIC should be recognised. The use of products containing factor IX could be hazardous in patients with a history of fibrinolysis, myocardial infarction, DIC or liver disease.
- There has been no clinical experience with MonoFIX-VF with respect to inhibitor development in previously untreated patients.
- Effect of heparin - As each vial of MonoFIX-VF contains 50-140 IU of heparin, consideration should be given to the clinical effect of heparin if the patient develops thrombocytopenia, if high doses of MonoFIX-VF are required, or if used during pregnancy/lactation.
Dosage and Administration
The exact loading and maintenance doses and dosing intervals should be based on the patient's clinical condition and response to therapy. Laboratory tests should be performed to ensure that the desired plasma factor IX concentration is achieved.
The dosage of MonoFIX-VF varies from 20-30 IU/kg for minor haemorrhage up to 100 IU/kg for major trauma or operative procedures.
The product can be given either as a bolus or by continuous infusion.
